Probility Gives Back

Probility Physical Therapy is committed to serving the community where we live and work. As a result, we have formed the Probility Gives Back program to support the community in the form of grants to nonprofit organizations and community events and programs.

Eligibility

To be eligible for contributions, organizations must:

• Qualify for nonprofit status under the Internal Revenue Code
• Benefit the local community served by Probility Physical Therapy
• Demonstrate sound and responsible financial policies and management

Contributions are made principally in the areas of education, the environment and human services; for example, assisting in health and housing and in combating hunger. Individuals are also eligible for contributions and should follow the application process below.

Application Process

After you complete the online request form, we will contact you. If you are eligible for a contribution, we will ask you to provide additional information, which may include:

• A description of your organization and project, including purpose and objectives, on agency letterhead signed by the Executive Director
• Proof of nonprofit status under Internal Revenue Code
• Compliance with the Equal Employment Opportunities Act and other federal, state and local laws and regulations
• Board of Directors list
• History of your organization

We will notify you of our decision in writing at the beginning of the next quarter. Contributions to a project imply no further commitment unless specifically stated at the time of the initial funding.
